Instructions: The SRA Questionnaire

This page contains step by step instructions for completing the SRA questionnaire. Please note that you should only complete the questionnaire once during any project and you should only complete the SRA if you have been invited to take part in the project.

  1. Click the link on the SRA Questionnaire Page to access the system or click here.
  2. You will be asked to enter your Project Identifier, which should have been provided to you by your administrator. Please enter this identifier and click [submit].
  3. You will then be asked to enter your first name and last name. Although these details are not mandatory, the information will be shown on your personal results. All personal identifying information is strictly private and confidential and your individual answers or personal results will not be seen by anybody else within the organisation.
  4. In each section of the questionnaire, you will see a rating scale. Please use this scale to answer each question according to how you have felt over the past four weeks. You should use your mouse to answer each question by clicking on a circle above the relevant number/answer. Once you have answered all the questions, please click [>>] to carry on to the next section. If you would like to go back to the previous section, you should click the backward arrow button [<<].

    Please note that if you click the [<<] button, although your answers to the previous sections will still be there, you will need to re-answer the questions as you move forward [>>] through the questionnaire.

    As some sections contain more questions than others, you may need to use the scroll bar on the right hand side of the window to answer all the questions.
  5. The final section, Section 12, asks for your suggestions for how your organisation could reduce the risks associated with workplace stress. Three boxes have been provided for you to type your comments. The system will limit your comments to about 50 words for each suggestion so please keep your suggestions brief and specific. You can press your tab key to move to each box, or simply use your mouse to click to each box.
  6. When you have completed this section, please click on [Your Results] to see your individual profile.
  7. The final window [Your Results] shows your ‘level of risk’ on each of the main scales measured by the SRA. There is a printer logo which you can click to print your results. You will also be given a new report identifier password, unique to you, which will enable you to view your results at a later stage. This report identifier can be sent to you if you enter your email address. Premier Occupational Healthcare will only use the email address you have submitted to us to send you your password. These personal details will not be retained by us, or passed on to third parties.
  8. To exit from the questionnaire please click the [x] button at the top right hand corner of the window or the exit button shown at the end of your results.
