Online Stress Risk Assessment
You have been invited to take part in a group pressure management project and complete the StressRisk Assessment Questionnaire (SRA).
The SRA questionnaire should take approximately 10 minutes to complete and you should base your answers on how you have felt during the last month. You will need a project identifier to take part. If you have not received a project identifier, please contact the administrator for this information. When you have completed the questionnaire, you will see your personal results and, when your colleagues have completed the questionnaire and the project has been closed, you will be able to see the group report.

The process is strictly confidential and nobody within your organisation will ever see your completed questionnaire or your individual results. Premier Occupational Health will not share any personally identifiable or private information submitted online with any third party.
Please note that you should only complete the SRA once. If you feel you have incorrectly completed the SRA, please contact your project administrator who will be able to delete your record and allow you to complete the SRA again. Please note that although the administrator will be able to see your name (if you have entered it), they will not be able to see your answers or results.
